How to complete and submit a form

2. Complete and save it
Open the downloaded PDF, complete all required fields, and save your completed copy.
3. Attach and email it
Use the Save PDF & Open Email button at the bottom of the form. When your email opens, attach the completed PDF and send it to AURA Admissions.
Important: Selecting the email button may open a new email without automatically attaching the completed PDF. Before sending, confirm that your saved form is attached.
Application for Admission
For parents or guardians applying for a student’s admission to AURA.
The application collects student information, current and previous school information, parent or guardian contact details, educational background, student interests, support information, required certifications, and signatures.
Recommended for: Families ready to submit a formal application for admission.
Four-page fillable PDF. A parent or legal guardian must complete and sign the application.
Statement of Interest
Use this form to tell AURA more about the student, the family’s interest in the school, educational history, interests, goals, and enrollment needs.
The form also requests student and parent or guardian information, enrollment-document information, school history, student-support information, household information, signatures, and agreement with the stated admissions provisions.
Recommended for: Families beginning the admissions process or providing detailed information about a prospective student.
Six-page fillable PDF. The applicant and a parent or guardian must complete the appropriate sections and signatures.
Teacher Recommendation
This confidential form should be completed by a current or recent teacher who knows the student’s academic abilities, work habits, classroom participation, conduct, and personal development.
The form includes student ratings, family-engagement ratings, narrative questions, an overall recommendation, teacher certification, and signature fields.
Recommended for: A teacher who has directly taught the student.
The teacher should complete and return this form directly to AURA Admissions whenever possible.
